Redoing Crowns With Gum Inflammation and Dark Margins: Periodontal Care First

October 29, 2026Redoing Crowns With Gum Inflammation and Dark Margins: Periodontal Care First

Initial condition

Ms. N.T.H. Minh (born 1970) had 18 cosmetic crowns placed about five years earlier. She later developed severe gum inflammation, bad breath, dark gum margins and unbalanced tooth shapes that had not improved despite several treatments.

Treatment plan

  • Periodontal treatment, including periodontal surgery (curettage), to stabilise the gums.
  • Replacement of the 18 crowns with Ceramil restorations and improved tooth shape.

According to the clinic's records, treatment was completed over 3 appointments and the patient was satisfied with the result.

Why can crowns cause gum problems?

  • Periodontal disease not treated before crowns are placed.
  • Poorly fitting margins or margins placed too deep under the gum.
  • Metal-based crowns that can show a grey line when gums recede.
  • Gum recession over time due to age, incorrect brushing or periodontal disease.
  • Difficult cleaning and lack of regular check-ups.

Crowns require removing natural tooth structure, so they should be properly indicated and carefully performed.

Note: Treatment indications and outcomes depend on each person's oral condition, general health and home care. This article is for general information only; an in-person examination is needed for individual advice.
